Responsibilities

The Home Health Patient Care Manager supervises and coordinates clinical services, providing nursing care to patients in their homes as prescribed by physicians and in compliance with laws, regulations, and agency policies.

• Provide clinical services within your scope of practice, as defined by state laws, according to the plan of care and in collaboration with other healthcare team members
• Manage referrals, assign clinicians appropriately, evaluate patient orders, and schedule start of care visits
• Coordinate the determination of patient benefits, medical necessity, and insurance approvals
• Continuously assess patient needs, ensuring care is individualized, appropriate, and meets eligibility criteria and physician orders
• Review assessments and care plans daily, consult with clinicians as needed, and make recommendations

Education and Experience

• Current RN licensure in the state of practice
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to a reliable vehicle or public transportation
