Responsibilities

The Home Health Patient Care Manager supervises and coordinates clinical services, providing nursing care to patients in their homes as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies.

• Provide clinical services within the scope of practice defined by state laws, according to the plan of care, and in collaboration with the healthcare team
• Receive referrals, assign clinicians appropriately, evaluate patient orders, and schedule start of care visits
• Coordinate patient home health benefits, medical necessity, and insurance approvals
• Continuously assess patient needs, ensuring care is individualized, appropriate, meets eligibility criteria, and follows physician orders
• Review assessments and care plans daily, consult with clinicians as needed

Education and Experience

• Current RN licensure in the state of practice
• Current CPR certification
• Valid Driver's License, vehicle insurance, and reliable transportation
