Responsibilities

The Licensed Practical Nurse (LPN) Patient Care Manager Assistant supports the Patient Care Manager by coordinating staff to ensure high-quality, continuous care for home health patients. This includes ensuring staff are skilled, informed, and committed to providing quality care in accordance with physician's orders and regulatory guidelines.

• Assist with communication from field staff, physicians, referral sources, and patients, including taking messages and orders
• Monitor incoming faxes and report information to relevant personnel
• Coordinate communication and document transmission with physician offices
• Track pending labs, facilitate follow-up, and communicate results to physicians and staff
• Monitor hospital patient status to ensure proper care coordination
• Assist with managing software reports for care coordination and utilization management
• Support payer source authorizations as needed
• Follow up on health check calls or tele-monitoring alerts, ensuring team communication
• Prepare patient lists and documents for weekly case conferences
• Provide direct patient care as needed, within scope of practice and physician orders, without altering the care plan
• Assist with clinician documentation reviews and audits to ensure accuracy and completeness

Education and Experience

License Requirements

• Current CPR certification
• Current LPN licensure in the state of practice
• Valid driver's license, vehicle insurance, and reliable transportation
