Job summary

Community Healthcare of Texas is seeking a Patient Care Manager to provide clinical oversight of direct patient care in a Dallas hospice setting. The PCM will lead staff, manage admissions, coordinate with physicians, and ensure quality patient outcomes in line with CHOT/Providence standards.

The role requires Texas RN licensure, a BSN preferred, and 2+ years hospice supervisory experience. Strong communication and scheduling skills are essential, with travel as needed in the service area.

Qualifications

  • Requires a college degree in nursing; BSN strongly preferred.
  • Requires 2+ years of hospice experience and supervisory experience.
  • Licensed RN in Texas.

Responsibilities

  • Oversee admission process for new patients and coordinate with hospice team.
  • Manage direct patient care by assigning staff, scheduling, and addressing issues.
  • Review patients' diagnosis, prognosis, and plan of care.
  • Assist clinicians in establishing goals and priorities and developing a plan of care.
  • Attend case conferences to coordinate care and interdisciplinary involvement.
  • Assist in screening and interviewing new employees and orientations.
  • Support clinical director in planning and education programs; help revise policies.
  • Audit patient charts and ensure compliance with regulations.
  • Maintain compliance with regulatory and accrediting agencies.
  • Participate in quality improvement teams and activities.
  • Hire, evaluate, coach, and schedule staff; delegate appropriately.
  • Educate and orient staff to agency policies.
  • Manage assigned patients equitably across cases.
  • Perform other duties delegated by the clinical director.

POSITION SUMMARY

The Patient Care Manager (PCM) is responsible for clinical oversight of all direct patient care, working through and with a variety of staff members, demonstrating the philosophy, mission, and service standards of CHOT/Providence.

ESSENTIAL FUNCTIONS
  • Oversee the admission process for new patients and confer with the team, hospice physician, or CMO as needed for guidance.
  • Manage all direct patient care by assigning staff to patients, scheduling staff for maximum budgetary optimization, including PTO and on-call, resolving patient care issues, visiting patients as needed, coordinate interdisciplinary groups.
  • Review patients' medical diagnosis, prognosis, medication, procedures, and clinical course.
  • Assist clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care.
  • Attend case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
  • Assist in the screening and interviewing process of new employees and make recommendations for employment of individuals. Assist in the orientation of new employees
  • Assist the clinical Director and other supervisory hospice personnel in the planning, implementation, and evaluation of in-service and continuing education programs. Assist in formulating, revising, implementing, and evaluating organization policies, procedures, goals, and objectives, both short and long-range.
  • Audit patient charts, respond appropriately to compliance issues, and write compliance/QA reports.
  • Maintain compliance with all regulatory and accrediting agencies.
  • Actively participate in quality assessment performance improvement teams and activities.
  • Manage personnel through effective hiring, performance evaluation, coaching, staffing, scheduling, and delegation.
  • Assist the Sr. Director as needed.
  • Educate, counsel, and orient staff to Agency policy/procedures.
  • Manage assigned patients equally, regardless of severity, contagiousness, or complexity of diagnosis.
  • Perform other duties and activities as delegated by the clinical Director.
QUALIFICATIONS
  • Requires a college degree in nursing; BSN strongly preferred
  • Requires 2+ years of hospice experience and supervisory experience
  • Experience in a Charge Nurse role will be considered to meet the supervisory requirement
  • Hospice experience and knowledge of hospice regulations strongly preferred
  • Proficient skills using a computer are required; Microsoft Office suite experience is preferred
  • Good communication skills and the ability to travel independently
  • Licensed RN in Texas required
  • Valid TX Driver's License required
