Job summary

Community Healthcare of Texas is seeking a Care Coordinator to serve as the hospice expert, coordinating services and bridging the patient transition to hospice care. You will develop relationships at facilities to position CHOT/Providence Hospice as the provider of choice.

The role requires 1–3 years in healthcare marketing and at least 1 year hospice experience, with strong clinical knowledge and proficiency in Microsoft Office to document referrals, educate staff, and support patient families.

Qualifications

• 1-3 years of healthcare marketing experience.
• 1 year of hospice experience.
• Medical knowledge strongly preferred (ability to read charts, know medications).
• Proficient using PC including Microsoft Office.

Responsibilities

• Respond to referral sources in a timely manner; evaluate patient desire for hospice and discuss services with families.
• Complete required paperwork with patients/families; facilitate discharge when needed with facility staff, DME, medications, and hospice providers.
• Educate facility staff and follow up on pending patients; be available to staff and patients for questions and to build relationships.
• Provide daily feedback to admission staff on referrals.
• Through customer service and leveraging relationships, promote CHOT/Providence Hospice as the hospice of choice and increase hospital referrals.
• Attend and participate in routine staff meetings in person or via provided media.
• Participate in in-service training and perform other duties as required.

Community Healthcare of Texas is currently recruiting for a Care Coordinator.

POSITION SUMMARY

Serve as the hospice expert to professionally and seamlessly coordinate services and bridge the transitions for the patient to hospice services; develop long-term relationships at each facility to reinforce CHOT/Providence Hospice as the provider of choice.

ESSENTIAL FUNCTIONS

• Respond to referral sources in a timely manner; evaluate patient desire for hospice; meet with families/patients to discuss goals and explain services of hospice.
• Complete required paperwork with patients/families; facilitate patient discharge when needed by working with facility/hospital staff, DME, medications, hospice physicians, and other facility staff.
• Educate facility/hospital staff and follow up on pending patients; be available to staff and patients for questions and to build relationships.
• Communicate and provide daily feedback to admission staff on referrals.
• Through customer service and leveraging relationships, become the “hospice of choice,” whereby increasing hospital referrals.
• Attend and participate in routine staff meetings in person or via media provided as directed.
• Participate in and/or provide in-service training, attend various facility/hospital activities and perform other duties as required or assigned.
