Community Healthcare of Texas is seeking a Care Coordinator to serve as the hospice expert, coordinating services and bridging the patient transition to hospice care. You will develop relationships at facilities to position CHOT/Providence Hospice as the provider of choice.
The role requires 1–3 years in healthcare marketing and at least 1 year hospice experience, with strong clinical knowledge and proficiency in Microsoft Office to document referrals, educate staff, and support patient families.
