Job summary

Community Healthcare of Texas is currently recruiting for a Care Coordinator in Dallas to professionally coordinate hospice services and bridge patient transitions to hospice care.

You will respond to referrals, complete paperwork, educate staff, and support admissions while building strong relationships with facilities to ensure CHOT is the provider of choice. This full-time role emphasizes customer service and collaboration in a healthcare setting.

Qualifications

• 1-3 years of healthcare marketing experience required.
• 1 year of hospice experience required.
• Medical knowledge strongly preferred (ability to read charts, know medications, etc.).
• Proficient using PC including Microsoft Office.

Responsibilities

• Respond to referral sources in a timely manner; evaluate patient desire for hospice; meet with families/patients to discuss goals and explain services of hospice.
• Complete required paperwork with patients/families; facilitate patient discharge when needed by working with facility/hospital staff, DME, medications, hospice physicians, and other facility staff.
• Educate facility/hospital staff and follow up on pending patients; be available to staff and patients for questions and to build relationships.
• Communicate and provide daily feedback to admission staff on referrals.
• Through customer service and leveraging relationships, become the "hospice of choice," whereby increasing hospital referrals.
• Attend and participate in routine staff meetings in person or via media provided as directed.
• Participate in and/or provide in-service training, attend various facility/hospital activities and perform other duties as required or assigned.

POSITION SUMMARY
