Job summary

Arcare seeks a Patient Care Coordinator to ensure comprehensive care coordination for HIV/AIDS clients. This role blends traditional case management with enhanced patient coordination services to support patients and families.

The PCC-Infectious Disease will collaborate with healthcare providers and other stakeholders to facilitate communication, manage care plans, and connect patients with resources, referrals, and insurance support.

Qualifications
