Job summary

Arcare is seeking a Patient Care Coordinator to ensure comprehensive, coordinated care for patients across their healthcare journey. The PCC will work closely with patients, families, providers, and stakeholders to facilitate communication, manage care plans, and provide resources and referrals.

Responsibilities include scheduling, transportation support, and connecting patients to community resources while tracking progress and addressing gaps to improve outcomes.
