Schedule: Monday–Friday | 8:00 AM–5:00 PM
Contract: 16-week assignment

About the Opportunity

We are seeking a compassionate and detail-oriented Patient Care Coordinator to support patients with outstanding specialist referrals. This role will focus on outbound patient outreach, referral follow-up, and coordination with specialist offices to ensure patients receive the care they need.

You will work through an assigned referral queue, communicate directly with patients and provider offices, and maintain accurate documentation throughout the process.

What You'll Do

• Work through an assigned queue of patients with open specialist referrals.
• Contact patients to determine whether they attended their scheduled specialist appointments.
• Communicate professionally and compassionately with patients regarding referral status and next steps.
• Document when patients decline or refuse referrals and notify the appropriate providers.
• Contact specialist offices to obtain consult notes, results, and other relevant medical documentation.
• Maintain accurate and timely documentation within the EMR.
• Follow established workflows and productivity expectations while working through a high-volume referral queue.
• Collaborate with providers and internal care teams to support continuity of care.
• Complete standard care coordinator training and apply training to daily referral activities.

Qualifications

Required:

• Previous referral coordination or clinical healthcare experience.
• Strong verbal and written communication skills.
• Comfortable conducting frequent outbound calls to patients and provider offices.
• Excellent attention to detail and documentation skills.
• Ability to manage multiple priorities in a fast-paced healthcare environment.

Preferred:

• Experience with eClinicalWorks or another EMR.
• Previous patient outreach, care coordination, scheduling, or referral management experience.

Monday–Friday, 8:00 AM–5:00 PM
