Position Overview

Patient Care Coordinators are integral members of the team responsible for delivering exceptional customer service to our patients. As a patient‑facing caregiver, you’ll provide customer service, helping patients through processes like check‑in, registration, and any questions relevant to their care. Coordinators work in conjunction with our clinical teams to assist with scheduling, clinic operations, and more.

This position is responsible for contributing to improved workflow processes, communications, and standards, as well as onsite initiatives for operational, financial, and clinical performance. The care team is dedicated to providing an environment and experience to our patients/families that convey our promise to them as well as Zarminali values.

Pay Rate

$16.00 – $19.00 per hour

What You’ll Do
  • Deliver an Exceptional Patient Experience
    • Provide a warm, welcoming, and patient‑first environment, ensuring that families feel valued and cared for.
    • Exhibit empathy, courtesy, competence, efficiency, and care in all interactions.
  • Seamless Scheduling & Coordination
    • Manage all patient appointments, including treatments, consultations, and referrals, ensuring seamless communication with families.
    • Coordinate schedules across multiple clinicians to optimize patient flow and minimize wait times.
    • Assist Practice Manager to maintain and update clinician schedule templates in real‑time to accommodate just‑in‑time scheduling needs.
  • Patient & Provider Communication
    • Serve as a key liaison between patients, caregivers, and clinical teams, ensuring clear and timely communication regarding appointments, treatment plans, and any additional needs.
    • Triage and direct calls from families, referring physicians, and other healthcare professionals, ensuring prompt and appropriate responses.
    • Monitor clinical inboxes and execute follow‑ups on role‑specific tasks efficiently.
  • Technology & Data Management
    • Develop expertise in the clinic’s electronic medical record (EMR) system and other healthcare technologies to efficiently manage patient data and workflow.
    • Accurately enter and update patient demographic, insurance, and contact details in the electronic health record (EHR) system.
    • Utilize multiple digital tools, phone systems, and patient portals to enhance communication and operational efficiency.
    • Assist with digital outreach efforts, including reminders for return‑to‑office visits, gaps in care, and quality metric initiatives.
  • Operational Support & Compliance
    • Ensure smooth clinic operations by assisting with intake processes, rooming patients, and obtaining/documenting vitals when necessary.
    • Manage administrative tasks such as scanning, faxing, mailing medical documents, and maintaining office cleanliness.
    • Adhere to insurance and referral protocols while advising patients of their responsibilities.
    • Recognize and respond appropriately to emergency situations following clinic protocols.
  • Collaboration & Continuous Improvement
    • Support cross‑coverage needs, special projects, and training of new staff members.
    • Engage in ongoing training to stay current with EMR updates, clinic technology, and workflow improvements.
    • Contribute to innovation and process improvements in a fast‑paced, tech‑driven environment.
    • Ensure opening and closing routines are followed for a well‑organized, high‑functioning clinic.
What You’ll Need
  • High school diploma or equivalent.
  • 1+ years of customer service or outpatient medical setting experience.
  • 1+ years of experience with basic computer software, including Microsoft Office suite.
  • Experience with electronic health/medical record systems or other scheduling and registration software.
  • Basic knowledge of medical terminology.
  • Prior experience providing a high level of customer service in a fast‑paced environment, including handling confidential data with discretion.
  • Ability to work independently, organize, and prioritize concurrent responsibilities and demand.
  • Ability to communicate verbally in person, on the phone, and in writing in a clear and concise manner.
  • Ability to solve complex problems and meet patient needs.
  • Ability to work in a high pressure, deadline‑oriented, and complex health care environment.
  • Ability to review work to identify and eliminate errors and omissions.
  • Ability to build relationships while maintaining appropriate boundaries.
