Qualifications

  • High school diploma or equivalent.
  • 1 year of experience in healthcare, human services, or customer service.
  • Current BLS/CPR certification (or ability to obtain within 90 days of hire).
  • Ability to complete FHCC Clinical Support Competency Program.
  • Strong interpersonal and communication skills.
  • Ability to work with diverse and underserved populations.
  • Basic computer proficiency.
  • Act 33 and 34 Clearances required.
  • Candidates must not be listed on LEIE or similar exclusion databases.

Responsibilities

  • Welcome patients and assist with the intake process.
  • Coordinate referrals to specialty care and community resources.
  • Assist patients in scheduling follow-up appointments.
  • Assist with prescription facilitation according to guidelines.
  • Reinforce provider instructions using educational materials.
  • Support patient navigation throughout the continuum of care.
  • Complete in-house Medical Assistant training by a licensed health care provider.
  • Escort and room patients.
  • Obtain and document patient vital signs.
  • Document activities in the electronic health record.
  • Communicate pertinent patient information to licensed providers promptly.
  • Maintain patient confidentiality per HIPAA.
  • Participate in team meetings and interdisciplinary care planning.
  • Support volunteers, students, and trainees.

We’re seeking a compassionate and organized Patient Care Coordinator in our Free Health Care Center in Downtown Pittsburgh.

The Patient Care Coordinator serves as a vital link between patients, healthcare providers, community resources, and clinic staff. Working under the direction of licensed healthcare providers, the Patient Care Coordinator assists with patient intake, basic clinical support services, patient navigation, and care coordination to promote efficient, high-quality care within the Free Health Care Center.

  • Support volunteer providers, students, and trainees as appropriate.
Requirements
  • A high school diploma or equivalent is required.
  • One year of experience in healthcare, human services, or customer service is required.
  • Current BLS/CPR certification (or ability to obtain within 90 days of hire) is required.
  • The ability to complete the FHCC Clinical Support Competency Program is required.
