Job summary

Elev8 Recovery Center in Harlem, NY is seeking a Patient Care Coordinator to support clients and reduce barriers to treatment. You will engage clients in addiction treatment services, model recovery competencies, and encourage hope for clients in recovery.

Responsibilities include coordinating with counselors and nursing staff, monitoring client care, and assisting with de-escalation and daily activity schedules. This is a full-time, on-site role offering hourly pay and shift-based census checks.

Qualifications

• High school diploma or equivalent
• 1 year of related work experience

Responsibilities

• Interact with clients in a professional, courteous, empathetic, and supportive way
• Regularly communicate with Counselors and Nursing staff regarding client care
• Encourage positive interactions and conversations amongst clients and staff
• Transport clients throughout the facility
• Ensure clients adhere to program activity schedule
• Conduct room checks on all clients
• Perform census checks every 2 hours (4 times per shift)
• Observe clients for inconsistent behavior and report to appropriate staff
• Assist in the de-escalation of behaviorally compromised clients
• Monitor client linen and laundry
• Monitor linen supplies and replenish as needed
• Ensure soiled linen is placed in laundry bin at the end of every shift
• Collect laundry bags of dirty clothing and prepare for housekeeping to wash
• Deliver clean laundry back to clients
• Search and record new admissions belongings
• Assist staff with screening the client and their property for contraband
• Performs other duties as assigned by supervisor
• Act as a role model for clients and peers

Description

Elev8 Recovery Center is located in Harlem, NY. We are a first-classinpatient treatment center offering top-level medical care and evidence-based clinical care for individuals suffering from substance use disorder. We are seeking Patient Care Coordinators to join our team. Patient Care Coordinator will be responsible for providing support to clients to reduce barriers to treatment and keeping clients engaged in addiction treatment services. They will model competency in recovery and ongoing coping skills while encouraging and inspiring hope for a client in recovery.
