What is a Patient Care Coordinator?

A day in the life of a Patient Care Coordinator:
- Greet everyone entering the clinic warmly.
- Schedule new referrals via fax or phone from patients and physicians.
- Verify insurance coverage for patients.
- Collect patient payments.
- Keep the front office organized and tidy.
- Perform other duties as assigned.

Position Summary:
The Patient Care Coordinator I (PCC-I) supports clinic growth by executing practice management and patient intake processes. Collaborates with the Clinic Director to ensure efficient clinic procedures, supporting Upstream Rehabilitation's mission, vision, and values.
Responsibilities:
- Collect all payments at the time of service.
- Convert referrals into evaluations.
- Schedule patient visits.
- Provide excellent customer service.
- Create a welcoming clinic environment.
- Make welcome calls and monitor patient arrival to enhance the customer experience.
- Manage schedules, documents, deposits, caseloads, and inventory efficiently.
- Attend required training sessions and complete quarterly compliance training.
Qualifications:
- High School Diploma or equivalent.
- Strong communication skills to relate well with leadership and team members.
- Ability to multitask, organize, meet deadlines, and work independently.
- Effective time management and presentation skills.
Physical Requirements:
- Primarily indoor environment; protected from weather but not temperature changes; noise level typical of an office.
- Full-time hours, Monday through Friday, with occasional evening or weekend work for events or additional hours.
- Sedentary work with frequent sitting and occasional standing, walking, and lifting (up to 20 lbs.).
- Must be able to communicate effectively, operate office equipment, and view computer screens constantly.
