Job summary

Optimal Health Chiropractic & Physical Therapy is hiring a Patient Care Coordinator to act as the quarterback of the patient journey in a fast-growing practice. You’ll connect patients with doctors, therapists, and front office, ensuring appointments are scheduled, understood, and kept.

You’ll build relationships, communicate clearly on the phone and in person, and manage scheduling, finances, and logistics.

The Mission

Our doctors and physical therapists can create an excellent plan for a patient. But a great plan means very little if the patient doesn’t understand it, doesn’t schedule it, cancels appointments, disappears halfway through care, or never follows through. Your job is to make sure patients don’t get lost. You’ll become the quarterback of the patient journey - connecting patients with our doctors, therapists, clinical team, and front office. You’ll know who needs a phone call. Who needs to get scheduled. Who cancelled. Who hasn’t followed through. Who has a question. Who needs to talk to their doctor. And who said, "I’ll call you later" three days ago and still hasn’t called. You don’t wait for those problems to find you. You find them first.


What You’ll Actually Do


  • Building genuine relationships with patients

  • Talking with patients in person and on the phone

  • Making outbound follow-up calls every day

  • Helping new patients navigate their first few visits

  • Supporting patients as they understand and commit to their treatment plans

  • Helping patients work through scheduling, financial, and logistical concerns

  • Following up immediately when patients begin falling off their care plan

  • Re-engaging patients who have disappeared

  • Keeping patients appropriately scheduled

  • Coordinating chiropractic and physical therapy care

  • Reinforcing recommendations made by our providers

  • Recognizing when a question needs to go back to a doctor

  • Tracking follow-ups so nobody falls through the cracks

  • Helping our team deliver an exceptional experience in a high-volume practice


There is administration involved. There are phones. There is scheduling. But those are tools of the job - not the purpose of the job. The purpose is to own the patient experience and help people follow through.

  • And you’re comfortable asking someone to make a decision.

  • You’re ridiculously organized. If you tell a patient you’ll call Tuesday, you call Tuesday. Nobody should need to remind you. You have some competitive fire. You like knowing whether you’re winning. Goals don’t scare you. Being measured doesn’t offend you. You’d rather know where you stand and figure out how to improve. You’re coachable.


We are building detailed training systems around patient communication, follow-up, sales, care-plan recovery, and the entire patient journey. We’ll teach you. But we’re going to expect you to learn it, practice it, and get good at it. You take ownership. This might be the most important one. When something becomes yours, it’s yours until it’s finished. Not: "I left them a voicemail." But: "Here’s where that patient stands, here’s what I did, and here’s what happens next." That’s the mentality we’re looking for.


Don’t Have Healthcare Experience?

That’s okay. In fact, we’re open to someone completely outside healthcare. We can teach you our systems. We can teach you chiropractic. We can teach you physical therapy. We can teach you the patient journey. We can teach you our approach to communication. What is much harder to teach is energy, curiosity, ownership, work ethic, confidence, and the ability to connect with people.

Your Schedule

This is a full-time, in-person position. Healthcare doesn’t always fit neatly between 9:00 and 5:00, and neither does this role. Our patient schedule includes some early mornings and evenings. Some shifts may run until approximately 6:00-7:00 PM, but you will not work late every night. Expect a mix of earlier and later shifts depending on patient volume and team coverage. Some evening flexibility is required.


Compensation

Competitive hourly compensation based on experience, demonstrated ability, and fit. There is also opportunity for performance-based bonuses after successful training and demonstrated competency.
