Job Summary
WELLBE INTRODUCTION

The WellBe care model is a Physician Led Advanced Geriatric Care Program focused on the quality of care for frail, poly-chronic, and elderly Medicare Advantage patients. This population is typically underserved and faces challenges accessing healthcare. We bring care directly to the patient across various settings including home, hospital, skilled nursing facility, assisted living, palliative, and end-of-life care. Our physician/advanced practice clinician-led geriatric teams partner with primary care physicians to deliver concierge-level medical care and social support, ensuring comprehensive care across the continuum.

General Summary

Our Patient Care Coordinators are part of a home-based care team that provides compassionate care to the frail population suffering from chronic and acute illnesses or injuries. Responsibilities include managing calls, coordinating care, scheduling appointments and tests, engaging patients, and maintaining accurate records. The ideal candidate is critical thinker, tech-savvy, flexible, and passionate about working with patients and a collaborative team. We foster a positive, supportive work environment focused on patient well-being.

Job Description
Skills & Competencies
  • Practices the WellBe mission: To help our patients lead healthier, meaningful lives by delivering the most Complete Care.
  • Maintains digital client records and assists in transitioning from hard copy to digital formats.
  • Acts as a community resource for patients.
  • Handles patient scheduling, registration, and eligibility verification.
  • Manages medical records appropriately.
  • Answers calls and emails, prioritizes patient concerns.
  • Explains processes and answers non-clinical questions.
  • Coordinates provider visits, tests, specialist appointments, and social resources.
  • Ensures all patient information is in the EMR.
  • Provides excellent customer service and respects all patients and staff.
  • Prioritizes urgent needs and efforts to engage unengaged patients.
  • Participates in team meetings and collaborates with interdisciplinary team members.
  • Supports panel management and quality of care efforts (e.g., HEDIS).
  • Prepares charts for next-day visits.
  • Uses reporting tools to help meet team goals.
  • Performs other tasks as needed.
Job Requirements
Educational/Experience
  • High school diploma or GED.
License, Certification, Registration
  • MA Certification preferred.
Required Skills & Abilities
  • At least 2 years of experience in healthcare practice or hospital setting.
  • EMR experience preferred.
  • Professionalism, critical thinking, customer service focus.
  • Strong computer skills (Word, Excel, PowerPoint).
  • Excellent verbal, written, and interpersonal communication skills.

Other details: No supervisory responsibilities. Travel up to 25% locally. Ability to lift up to 20lbs, with some tasks involving lifting heavier weights, standing for extended periods, and driving to patient locations.
