Overview

WellBe Introduction

The WellBe care model is a Physician-Led Advanced Geriatric Care Program focused on providing quality care to frail, poly-chronic, and elderly Medicare Advantage patients. This underserved population faces challenges in accessing healthcare. Our approach is to bring care directly to the patient across various settings, including the home, hospital, skilled nursing facility, assisted living, palliative, and end-of-life care. Our physician and advanced practice clinician-led teams collaborate with primary care physicians to deliver concierge-level medical and social support, ensuring comprehensive care across the continuum.

Role Summary

Our Patient Care Coordinators are integral to our home-based care teams, providing compassionate care to patients with chronic and acute conditions. Responsibilities include managing calls, coordinating care and appointments, engaging with patients, and maintaining accurate records. The ideal candidate is thoughtful, tech-savvy, flexible, and passionate about working with patients and interdisciplinary teams. We foster a positive, collaborative work environment dedicated to patient well-being.

Skills & Competencies

  • Maintain and update digital records; assist in transitioning paper records to digital formats.
  • Serve as a community resource for patients.
  • Schedule and coordinate patient and specialist appointments, tests, and social resources.
  • Manage patient records and ensure accurate documentation in EMR.
  • Provide excellent customer service, addressing patient inquiries professionally.
  • Prioritize urgent needs and efforts to engage unengaged patients.
  • Collaborate with the interdisciplinary team and participate in meetings.
  • Support panel management and quality initiatives like HEDIS.
  • Prepare for next-day visits and utilize reporting tools to track team goals.
  • Perform additional tasks as needed to meet team objectives.
Requirements

Education & Experience:

  • High school diploma or GED.

Certifications:

  • MA Certification (preferred).

Skills & Abilities:

  • At least 2 years of healthcare practice or hospital experience.
  • Experience with EMR systems preferred.
  • Professional demeanor, critical thinking, customer focus, and strong communication skills.
  • Proficient in Word, Excel, PowerPoint.
Additional Details

Travel may be required up to 25% locally. Must be able to lift up to 20 lbs, with potential to assist with heavier weights. Ability to stand for extended periods and drive to patient locations is necessary. Reasonable accommodations will be made to enable performance of essential functions.
