WellBe Introduction
The WellBe care model is a Physician-Led Advanced Geriatric Care Program focused on providing quality care to frail, poly-chronic, and elderly Medicare Advantage patients. This underserved population faces challenges in accessing healthcare. Our approach is to bring care directly to the patient across various settings, including the home, hospital, skilled nursing facility, assisted living, palliative, and end-of-life care. Our physician and advanced practice clinician-led teams collaborate with primary care physicians to deliver concierge-level medical and social support, ensuring comprehensive care across the continuum.
Our Patient Care Coordinators are integral to our home-based care teams, providing compassionate care to patients with chronic and acute conditions. Responsibilities include managing calls, coordinating care and appointments, engaging with patients, and maintaining accurate records. The ideal candidate is thoughtful, tech-savvy, flexible, and passionate about working with patients and interdisciplinary teams.
