Position Summary: As a Patient Care Coordinator, you'll serve as the operational hub connecting patients, healthcare providers, and our mobile clinical teams. Every day, you'll coordinate patient visits, prioritize urgent needs, optimize clinical schedules and routes, and ensure our patients receive timely, high‑quality care. Success in this role requires strong multitasking skills, clear communication, critical thinking, and the ability to make real‑time decisions in a dynamic healthcare environment. In addition to scheduling and coordination, PCCs help support daily office operations by assisting with equipment management, inventory tracking, and fleet coordination.
