Job summary

Evara Health is seeking a Patient Care Coordinator in Clearwater, FL to manage appointment scheduling, patient tracking, and care coordination for behavioral health and primary care services.

This full-time, on-site role supports the Prevention Case Manager, processes patient records, and ensures regulatory compliance while enhancing patient access to resources and services offered by our network.

Qualifications

• Minimum of High School Diploma or GED.
• Bachelor’s degree in health or related field preferred.

Responsibilities

• Appointment Coordination & Tracking – Schedule and monitor medical and behavioral health appointments, track patient progress, and support provider tasks and orders.
• Care Coordination & Case Management Support – Assist the Prevention Case Manager with patient care coordination, follow-up, and addressing barriers to treatment.
• Documentation & Communication – Process and manage patient records, referrals, and provider orders, including scanning, faxing, and routing documents.
• Regulatory Compliance & Customer Service – Ensure adherence to policies, maintain high-quality patient interactions, and facilitate connections to resources that address Social Determinants of Health.
• Team Collaboration & Additional Duties – Work collaboratively with colleagues, provide mentorship, and support organizational goals as needed.
