Hebrew SeniorLife in Roslindale (Boston area) seeks a Patient Care Associate to support residents with activities of daily living, skincare, meals, and mobility. You will work as part of a caring team to promote a home-like environment and high-quality, safe care.

Ideal candidates hold CNA certification or are enrolled in nursing school, have fluent English skills, computer literacy, and a collaborative, flexible approach.

Qualifications

• Long term chronic care PCA CNA certification preferred in good standing upon hire or current enrollment in nursing school.
• RSU PCA: CNA certification required in good standing upon hire and ongoing.
• Fluent English skills required – verbal and written.
• Computer literacy required.
• Flexibility and ability to work independently and collaboratively.
• Must complete mandatory HSL/HRC educational requirements and programs.

Responsibilities

• Recognize and respond to patient and family needs and communicate changes to the nursing team.
• Adhere to Safe Patient Handling practices and maintain a no manual lift environment.
• Assist patients with personal care as indicated in care plans and document care delivered in EMR.
• Report accidents, incidents, abuse, and observe skin integrity.
• Participate in daily report, care planning meetings, and infection control practices.
• Maintain confidentiality per HIPAA; communicate effectively with peers, patients, and families.
• Facilitate life enrichment activities and patient engagement; maintain room and program area cleanliness.
• Assist with meals, snacks, and meal cleanup; participate in committee meetings as needed.
• Perform vitals and basic procedures (temperature, pulse, respirations, BP, weight, etc.).
• Other duties as assigned and trained.

Position Summary:The primary role of the Patient Care Associate (PCA), is to protect, sustain, and nurture the patients that live at the Center. They provide assistance with activities of daily living, care of the patient’s environment, and meet other needs as required. The goal of the work of the Patient Care Associate, is to provide assistance that promotes a high quality of life for the patients.
