Job summary

The Admission Coordinator at Singing River Health System Pascagoula Hospital is a key member of the care team, supporting patients’ social and emotional needs and discharge planning. This role contributes to development of comprehensive care plans and ensures services align with facility policies and regulatory requirements.

Responsibilities include policy development for social services, participation in performance improvement plans, and completing operational tasks assigned by Nursing

Qualifications

• Completion of an accredited LPN or RN program
• Current Mississippi license to practice as LPN or RN
• Must complete and maintain BCLS certification upon hire and/or transfer
• 1+ years of social services experience; 5 years in long-term care/facility preferred

Responsibilities

• Develop patients' comprehensive care plans and discharge planning.
• Develop policies and procedures to provide social services in compliance with regulations.
• Oversee performance improvement plans, tracking and implementation.
• Perform operational tasks assigned by Nursing Leadership to support the Skilled Nursing Facility
