  • Facilitate access to mental health, substance use, and community support services
  • Maintain accurate documentation of patient interactions and advocacy efforts
  • Performs other duties as assigned by the Director of Quality and Risk Management
Requirements:
  • Experience in behavioral health, social services, case management, or a related field
  • Strong knowledge of mental health and substance use treatment systems
  • Excellent communication, interpersonal, and conflict resolution skills
  • Ability to maintain confidentiality and adhere to HIPAA regulations
  • Empathetic, patient-centered approach with a commitment to advocacy
  • Proficiency in maintaining accurate records and documentation
  • Bilingual (English/Spanish) a plus in the Albuquerque community
