Job summary

Sea Mar Community Health Centers seeks a Pathways Community Health Worker to connect patients to health and social services outside the clinic, delivering home and community-based support. You will document interactions and progress in Sea Mar’s Pathways platform and collaborate with HUB partners to address barriers to care.

The CHW will engage with clients, assess needs, and assist with program goals using motivational interviewing and culturally appropriate outreach in a diverse community

Qualifications

• Must be a trusted community member and communicate effectively in writing and speech.
• Proficient in using CCS Platform for documentation and resource referrals.
• Bilingual English/Spanish preferred; other languages a plus.

Responsibilities

• Work primarily in community/home settings to reach patients.
• Advocate for patients and connect to community resources.
• Document interventions and assessments promptly in Pathways and Sea Mar EHR.
• Attend Pathways trainings and collaborate with HUB partners.
