Job summary

Seeds of Hope in Georgia is seeking a licensed RN with home health experience to implement individual care plans and perform periodic evaluations. The role includes weekly in-home visits and monthly physical assessments, with follow-up on appointments, prescriptions, and discharge orders.

Documentation of services must be timely, and additional duties may be assigned. The ideal candidate has a current RN license, at least two years in home health, and experience caring for special needs

• Experience working with special needs population.
• Knowledge of current home management and personal care methods.

Responsibilities

• Implement individual care plans.
