Job summary

Seeds of Hope in Georgia is seeking a Registered Nurse to implement individualized care plans in a group home setting for individuals with developmental disabilities. You will conduct face-to-face evaluations, perform weekly visits, follow up on appointments and prescriptions, document care, and update healthcare plans as needed.

The role requires an active RN license, 2 years in home health, and experience with special needs populations.

Qualifications

• Current RN License
• 2 years of experience in home health or related field
• Experience working with special needs population
• Knowledge of current home management and personal care methods

Responsibilities

• Ensure the individual care plans are implemented
• Conduct a face-to-face evaluation of individuals as needed
• Make weekly home visits
• Follow-up with appointments, prescriptions, physician orders, discharge orders, etc.
• Document service provided to each individual in a timely manner

Additional Information

Basic Job Requirements Applicants must submit to a criminal background check before a an offer is extended (Employer Paid).

Other basic job requirements are as follows:

• Must be able to pass a Criminal Background Check
• Tuberculosis Screening Test Results
• Physical
• CPR/First-Aid
• Work 5-10 hours a week(paid direct deposit weekly)
• Recent 7 year Motor Vehicle Report copy if hired
• Complete online orientation and annual training
