Job summary

Job Description

• Ensure the individual care plans are implemented
• Conduct a face‑to‑face evaluation of individuals as needed
• Make weekly home visits
• Follow‑up with appointments, prescriptions, physician orders, discharge orders, etc.
• Document service provided to each individual in a timely manner
• Other related duties as assigned

Qualifications

Current RN License

2 years of experience in home health or related field

Experience working with special needs population

Knowledge of current home management and personal care methods

Additional Information
