Job summary

Find is seeking a full-time & part-time Partner Nurse Practitioner or Physician Assistant to provide care in assisted living, group homes, and skilled nursing facilities. The role involves diagnosing and treating patients through house call services, along with prescribing medications and collaborating with other healthcare professionals. Candidates must have an active Arizona license, experience as a primary care provider, and excellent communication skills.

Attractive compensation includes a base salary, per-visit percentage, and potential company equity. Preference for those who can travel within the Southeast or West Valley.

Responsibilities

• Diagnose and treat patients in assisted living and skilled nursing facilities.
• Prescribe necessary medications and treatments.
• Perform comprehensive physical examinations of patients.
• Develop and implement patient management policies.
• Facilitate referrals to other healthcare professionals.
• Communicate with collaborating physicians regarding patient care.

Skills

Excellent written and verbal communication skills

Ability to build excellent rapport with patients

Experience working in fast-paced environments

Experience with interdisciplinary teams

Bilingual is a plus

Education

Active Arizona License in good standing

Multiple years of experience as a Primary Care Nurse or Physician Assistant
