Job summary

HealthOp Solutions seeks a full-time Nurse Practitioner or Physician Assistant for post-acute care across Mesa and Chandler, AZ. You will provide direct patient care in skilled nursing and assisted living settings with weekday on-site hours and occasional weekend rotations.

Qualifications

• Active, unrestricted Arizona NP or PA license.
• Graduate of an accredited U.S.-based NP or PA program.
• 1–2 years of post-acute, long-term, or primary care experience preferred.

Responsibilities

• Provide direct, in-person care at SNF/ALF facilities.
• Maintain an on-site presence daily.
