Job summary

Jobtailor is seeking a dedicated professional to manage a caseload within our CFSS program in Minnesota. You will assess participant goals, develop Service Delivery Plans, educate on service models, and coordinate care with agencies to ensure compliance and quality.

Strong budgeting, documentation, and communication skills are essential. You will collaborate with staff to ensure SDP adherence, address ADLs/IADLs, and provide resources to promote participant independence while maintaining HIPAA

Qualifications

• Bachelor’s degree in a human services field or one year of direct service experience.
• Proven creativity, critical thinking, and problem-solving in diverse settings.
• Ability to lead with a person-centered approach and manage ADLs/IADLs.
• Ability to create and manage service budgets and allocate funds effectively.
• Strong relationship management skills between individuals and organizations.
• Excellent organizational, communication, and interpersonal skills.
• Ability to meet deadlines and leverage resources for service delivery.
• Willingness to learn Minnesota-specific regulations and use multiple software applications.

Responsibilities

• Assess participants’ goals and design SDP addressing diverse needs.
• Educate participants on service delivery models and CFSS program.
• Write or assist SDP documents aligned with goals and accommodations.
• Understand medical conditions to guide appropriate care for participants.
• Coordinate with staff and agencies to ensure SDP compliance and quality.
• Provide resources to participants and representatives to promote independence.
• Manage budgets to cover services, goods, fees, and taxes over the service period.
• Document interactions clearly in client management systems.
