Qualifications

Responsibilities

• Provides patient care to Wound Care Center patients and handles intake and discharge processes.
• Assist in vein clinic as assigned after training.
• May serve as Case Manager for a group of patients.
• Performs assessment, documents care plans, tracks wound status, and collaborates with physicians.
• Collaborates with healthcare providers to address clinic and patient needs.
