CareMed Health Solutions seeks a Wound Care Nurse Practitioner to deliver wound care services to residents in long-term care facilities in Ohio. This full-time role requires state licensing, board certification, and experience with chronic wounds and wound care products.

You will diagnose and treat wounds, prescribe medications per OH law, coordinate with facility staff and primary care physicians, and work within an interdisciplinary team using EMR to optimize patient outcomes.

Formación

• Wound care background including chronic, debilitating, stalled, or infected wounds.
• Comfort diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Ability to stage pressure injuries according to NPIAP guidelines.
• Experience with wound vacs and other DME (pressure-relieving devices).
• Experience in a long-term care setting with EMR usage.

While an interdisciplinary care team provides medical care to patients in long-term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Ohio.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care.
