Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to provide wound services to residents in long-term care facilities. You will prescribe medications, order and follow up on treatments, and collaborate with the interdisciplinary care team and primary care physicians.

Qualified candidates hold an Ohio NP license with board certification, experience in wound care and long-term care, and familiarity with EMR systems and wound care products. DEA licensure is optional.

Qualifications

• Must be licensed as an NP in Ohio.
• Board-certified NP with wound care experience.
• Familiar with wound care best practices and supplies.
• Experience with wound vacs and DME.
• Comfort diagnosing and treating chronic wounds including Kennedy ulcers.
• Experience in long-term care settings and EMR usage.
• Value collaboration within an interdisciplinary care team.

Responsibilities

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe medications, including controlled substances per OH law.
• Order, review, and follow up with treatments and diagnostics.
• Communicate with nursing home staff about changes in condition and care plans.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES:

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

Wound Np Qualifications And Education Requirements

• Current, unrestricted NP license in the state of Ohio.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care
