Job summary

CareMed Health Solutions seeks a Wound Care Nurse Practitioner to deliver wound services for residents in long-term care facilities, with emphasis on comprehensive assessment, treatment planning, and collaboration with facility staff and primary care providers.

The role requires OH licensure, board certification, and experience with wound care products and devices. Part-time schedule with benefits is offered on-site.

Qualifications

• Wound care expertise including chronic, acute, and Kennedy ulcers.
• Active, unrestricted NP license in Ohio.
• Board-certified with optional DEA licensure.
• Experience in long-term care settings and with wound VACs.

Responsibilities

• Provide wound care for long-term care residents in skilled nursing facilities.
• Prescribe medications, including controlled substances per OH law; order tests.
• Coordinate with nursing home staff on condition changes and care plans.
• Collaborate with the primary care physician and interdisciplinary team.
• Utilize EMR to document care and follow up on treatments.
• Engage in plan of care and outcomes in patient-centered approach.

While an interdisciplinary care team provides medical care to patients in long-term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of OH.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care.
