Job summary

UnitedHealth Group is seeking a part-time Weekend Shift Nurse Case Manager to guide patients and families through the care continuum. You will assess needs, coordinate care, support discharge and transition planning, and manage ongoing outpatient care under supervision.

Qualifications

• Current RN license required for the state you work.
• Current CPR certification required.
• 1+ years of clinical experience as an RN.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and services.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
