Join us at 21st Century Home Health Services as a Part Time / Weekend Physical Therapist Case Manager in the Sacramento Area. This role requires you to visit patients' homes, evaluate their physical health, and develop tailored care plans to help them regain independence and improve mobility. You will work in collaboration with other clinicians and healthcare providers, supervise Physical Therapy Assistants, and ensure compliance with regulatory standards.
Annual salary ranges from $120,000 to $135,000 plus a $10,000 quarterly bonus. Opportunities for promotion to PTII with higher salary and bonus are available.
