Part Time / Weekend Physical Therapist Case Manager: Home Health

Position Overview

This role requires you to visit patients' homes, evaluate their physical health, and develop tailored care plans to help them regain independence and improve mobility. You will work in collaboration with other clinicians and healthcare providers, supervise Physical Therapy Assistants, and ensure compliance with regulatory standards.

Responsibilities

1. Perform evaluations and develop care plans in patients' homes.
2. Assist patients with mobility and independence.
3. Create and adjust treatment plans.
4. Coordinate with physicians and report patient progress.

Qualifications

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources and healthcare regulations
• Experience in home health care and with high-acuity patients is preferred

Benefits and Compensation

Annual salary ranges from $120,000 to $135,000 plus a $10,000 quarterly bonus. Opportunities for promotion to PTII with higher salary and bonus are available.
