Position Overview

This role requires field visits to patients' homes within the Rocklin area. You will evaluate and provide skilled physical therapy services, helping patients regain independence and improve mobility. Supervision of Physical Therapy Assistants (PTAs) may be required, adhering to agency standards.

Key Responsibilities

1. Perform evaluations and develop care plans in patients' homes.
2. Assist patients with mobility and movement to promote independence.
3. Create detailed treatment plans tailored to each patient.
4. Coordinate with physicians to ensure quality care.
5. Report patient progress to physicians.

Qualifications

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR certification.
• Valid CA Driver's License.

• Experience in home health care and with high-acuity patients is preferred.
• Strong interpersonal, organizational, and leadership skills.

Compensation

Annual salary ranges from $120,000 to $135,000, with potential bonuses. Opportunities for promotion to PTII with increased responsibilities and salary.
