Responsibilities

• Maintain and submit accurate records (financial reports, behavior supports, personal care tasks, medication assistance).
• Document person-centered observations and quotes in CareLogic.
• Provide community living support per ISP, including advocacy, daily living skills, and home care tasks.
• Escort individuals to engage in community activities and promote inclusion.
