Job summary

McCormick Post Acute is seeking a part-time Speech-Language Pathologist to join our skilled nursing and rehabilitation team. You will evaluate residents with speech, language, cognitive, communication, and swallowing disorders and develop individualized treatment plans.

Qualifications

• Master's degree in Speech-Language Pathology required or equivalent.
• Current state license for Speech-Language Pathology.
• CCC-SLP certification preferred.
• Experience in skilled nursing or rehabilitation settings preferred.
• Excellent communication and interpersonal skills.
• Passion for helping residents achieve independence.

Responsibilities

• Evaluate residents with speech, language, cognitive, and swallowing disorders.
• Develop individualized treatment plans based on resident needs and goals.
• Provide skilled speech therapy services per physician orders and care plans.
• Educate residents, families, and caregivers on therapy goals and techniques.
• Collaborate with interdisciplinary team to optimize patient outcomes.
• Complete timely documentation and participate in care planning and discharge planning.
