Evolve Therapy Services, LLC is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Florida. This Part Time or PRN role focuses on delivering quality speech therapy services and coordinating care within CMS guidelines.

The position requires a Master’s degree in Speech Language Pathology, CCC-SLP, and an active state license. Continued professional development is encouraged to maintain licensure and certification, with benefits based on status.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals following standards and procedures.
• Evaluate and obtain data for treatment planning and implementation.
• Develop a treatment plan with target list, short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to ensure resident needs are met timely and with clinical excellence.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice
