Job summary

Evolve Therapy Services is seeking a licensed Speech Language Pathologist for an in-house Skilled Nursing Facility in Florida. The position is part-time or PRN, offering flexible scheduling to support patient care at the facility.

You will plan, implement, and document speech-language pathology services, coordinating with administrators and the site team to ensure high-quality resident care and CMS-compliant records.

Qualifications

• Master's degree in Speech-Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide quality speech therapy services for residents in the facility.
• Evaluate and collect data for treatment planning and progress.
• Develop treatment plans with short- and long-term goals.
• Instruct patients, families, and staff on speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and site program coordination.
• Participate in residents screening to identify needs and ensure timely care.
