Job summary

Evolve Therapy Services is seeking a compassionate Speech Language Pathologist for an in-house Skilled Nursing Facility located in Pahokee, FL. This Part Time or PRN position focuses on planning, delivering, and coordinating speech-language pathology services in accordance with applicable standards to ensure the highest quality resident care.

The role requires a Master’s in Speech Language Pathology, CCC and an active SLP license in the practice state, with ongoing continuing education.

Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency (CCC)
• Active SLP license in state(s) of practice
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate
• Evaluate and obtain data for treatment planning
• Develop treatment plans with target, short-term and long-term goals
• Instruct patients, families, and staff to ensure carryover of treatment goals
• Complete CMS-compliant documentation in NetHealth EMR
• Oversee FEES/MBS protocols and coordinate site programs
• Participate in screening protocols to ensure timely identification of resident needs
