Job summary

Beachwood Post Acute & Rehab is seeking a part-time Speech-Language Pathologist to evaluate and treat residents, develop individualized treatment plans, and ensure accurate documentation in the medical records. The role involves collaboration with physicians, nursing, families, and the healthcare team to support communication, swallowing safety, and discharge planning.

Qualifications

• Bachelor’s degree in Speech-Language Pathology (Masters/Doctorate preferred).
• Licensed as a Speech-Language Pathologist; continuing education per state requirements.
• Proficiency with Net Health and Point Click Care for documentation.

Responsibilities

• Evaluate residents to determine speech therapy needs and rehabilitation potential.
• Develop and implement individualized treatment plans; monitor progress and adjust as needed.
• Provide training and supervision to CFYs or students as applicable.
• Communicate progress with physicians, nursing staff, residents, and families; assist with discharge planning.
• Complete accurate documentation in accordance with regulatory and payer requirements.
