Job summary

Carease Health is seeking a Care Companion/Social Worker in Wisconsin to support residents in skilled nursing facilities. You will provide emotional support, brief therapeutic interventions, and mental health assessments as part of a Collaborative Care team.

The role involves home and facility visits, coordination with psychiatric professionals, and regular supervision. This is a part-time position with approximately 10 hours per week, Monday through Friday, within 8 am–5 pm hours.

Qualifications

• Geriatric mental health understanding and crisis management in long-term care settings
• Strong communication, documentation, and relationship-building skills
• Education: Associate Degree
• Experience: 1 year

Responsibilities

• Visit designated facilities according to the schedule with supervisor
• Assess residents to identify emotional, cognitive, and psychosocial needs
• Provide short-term, solution-focused therapy and supportive interventions
• Document visit notes and maintain detailed records after each visit
• Collaborate with psychiatric professionals, including participation in a monthly meeting with the psychiatric NP
