Qualifications

Responsibilities

• Visit residents upon admission to assess social, emotional, and environmental needs.
• Prepare and maintain documentation as regulated by authorities.
• Participate in care plan meetings and ongoing evaluation of needs.
• Monitor adherence to care plans and coordinate information flow with residents and families.
• Provide counseling and emotional support related to health and social needs.
• Act as liaison between residents, families and professionals.
• Coordinate discharge planning and transfers within communities.
• Assist placement in alternative care facilities when necessary.
