Job summary

Westerville Post Acute in Westerville, OH is seeking a part-time Social Worker to support residents’ emotional and social needs. The role is 2 days per week with a flexible schedule, designed to accommodate availability.

You will assist in planning, implementing, and evaluating social services in accordance with regulations and facility policies. Responsibilities include meeting with staff and families, coordinating resources, discharge planning, and maintaining progress notes.

Qualifications

• Ability to read, analyze, and interpret general business materials.
• Ability to write reports and communicate clearly.
• Ability to present information to managers and staff.

Responsibilities
