Job summary

Edgefield Post Acute is seeking a part-time Social Worker to join our team 2 days per week with a flexible schedule. The role focuses on planning, developing, and directing social service programs to meet residents’ emotional and social needs while ensuring compliance with federal, state, and local standards.

The position emphasizes collaboration with administration, medical and nursing staff, and other departments, aiming to improve resident well-being and access to community resources.

Qualifications

• Must possess a high school diploma or equivalent; associate/bachelor’s preferred but not required.
• Healthcare experience preferred.
• Ability to read, analyze, and interpret general business materials.
• Ability to write reports and communicate effectively with staff and residents.

Responsibilities

• Assist in planning, developing, and directing social service programs for residents.
• Meet with administration and medical staff to plan social services.
• Develop and coordinate department policies and procedures.
• Maintain social service progress notes and respond to residents’ needs.
• Provide information on Medicare/Medicaid and financial assistance options.
• Coordinate with other departments to meet residents’ social needs.
• Assist with discharge planning and community resource referrals.
• Support residents in achieving self-care and well-being goals.
