Job summary

Carease Health - OH is seeking a Social Worker/Care Companion to support emotional and mental well-being of residents in skilled nursing facilities. You will provide brief therapy, conduct assessments, and document interactions to ensure compassionate, person-centered care.

You will join a collaborative care team with regular supervision and monthly clinical meetings, offering training and growth opportunities.

Qualifications

• Associate degree required and understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills are essential.
• 1 year of experience in a similar role or settings.

Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
