Job summary

FreedomCare in Ohio is seeking a Part-Time Licensed Social Worker (LSW) to support the Structured Family Caregiving program in Ohio. This hybrid role requires fieldwork and a commute to the Cleveland office at least twice a month.

You will conduct in-person assessments, ensure plan of care compliance with HIPAA and state regulations, monitor patient enrollment and caregiver support, and serve as the escalation point for quality of service within the SFC program.

Qualifications

• Licensed Social Worker (LSW) in Ohio, active and valid.
• Bachelor’s or Master’s in Social Work from a CSWE-accredited program.

Responsibilities

• Conduct in-person home assessments with patients and caregivers.
• Monitor care plans and ensure HIPAA/compliance with Ohio regulations.
• Coordinate enrollment and caregiver support within the SFC program.
• Provide escalation and advocacy for quality of service as needed.
