Job summary

Eastgatespring of Cincinnati is seeking a part-time social services assistant to support the LSW/Social Service Director in delivering social services to residents and their families in Cincinnati, OH. The pay rate is $16.50–$20.00 per hour.

Responsibilities include assisting with assessments, discharge planning, care conferences, and routine documentation under supervision. Candidates should be STNA or eligible with long-term care experience.

Qualifications

• STNA or eligible and experience in long-term care.
• Approved by LSW and facility administrator.

Responsibilities

• Provide social services to residents and families under the direction of licensed social worker to help with adjustment process, identifying social and emotional needs, problem solving and financial issues.
• Assist LSW with gathering information for the Integrated Personal Services Assessment and MDS.
• Assist with discharge planning by arranging community resources and alternative placements.
• Attend Care Conferences as directed by the LSW.
• Fulfill basic documentation requirements for social services.
• Assist after training in the BIMS and PHQ-9.
