You will work closely with a multidisciplinary team to evaluate, plan, and implement individualized treatment plans aimed at maximizing independence and quality of life.

Responsabilidades

• Assess and treat speech, language, cognitive, communication, and swallowing disorders
• Develop individualized care plans based on resident needs and goals
• Collaborate with nursing, therapy, and interdisciplinary team members to ensure coordinated care
• Monitor progress and adjust treatment approaches as appropriate
• Educate residents, caregivers, and team members regarding treatment strategies and recommendations
• Maintain accurate and timely clinical documentation
