Job summary

Hopewell Center, Inc. in Indiana seeks a Part-Time Registered Nurse (RN) to support Wellness Coordination and HCBS services. The RN will oversee medication administration, provide clinical guidance, and train staff to maintain health and independence.

The role requires an Indiana RN license, strong clinical judgment, and collaboration with physicians and direct support teams. Some in-home visits and travel between Hopewell locations may be required.

Qualifications

• Graduate of an accredited nursing program with an active Indiana RN license in good standing.
• Strong clinical judgment to assess health concerns and interpret orders.
• Ability to train, coach, and provide nursing guidance to non-licensed staff.
• Willingness to obtain credentials related to medication-administration training.
• Ability to travel between Hopewell locations with a valid driver’s license.

Responsibilities

• Provide nursing oversight for Wellness Coordination and medication-administration within Supported Living/HCBS programs.
• Provide medication-administration training, competency support, staff education, and ongoing nursing guidance.
• Conduct nursing assessments, monitor health status, and complete quarterly summaries and follow-up.
• Coordinate with physicians, healthcare providers, Supported Living staff, QIDPs, and other Hopewell team members to support continuity of care.
• Review medical orders, address discrepancies, and provide clinical follow-up related to hospital visits, medication changes, seizures, injuries, and other significant healthcare concerns.
• Partner with Residential Services Director, QIDPs, and direct support teams to provide nursing consultation and safe health service implementation.
• Maintain required nursing documentation, monitor health trends and risks, and identify opportunities to improve health, safety, and quality of care.
